Welcome to Park Road Surgery
Our mission is to provide an effective healthcare service in order to meet the medical needs of our patients, and encourage health promotion to improve the health of our patients.
Summary Care Records (SCR)
The SCR or NHS Spine will soon be moving to the data collection stage. This means that some of your medical information with be uploaded onto the national spine for access by any health profession in contact with you across the country. You will soon be written to by Surrey PCT telling you more about it.
If you DO NOT want your information to be included on this spine you must opt out. This can be done either by telling you GP (in writing), or by completing an official form avilable through the link below - and then handing this to reception at the surgery or posting it in.
